To conclude an agreement for the provision of mobile communication services, the following documents will be required:

1. Original application for connection (on standard Mobiuz letterhead, or on the organization’s letterhead) 2. Copy of Certificate of Registration (License / Guvohnoma) 3. A copy of the Certificate of State Registration as a value added tax payer. 4. Original passport of the head of the company or a proxy (a copy of the manager’s passport is required) 5. Power of attorney for an authorized representative

For foreign missions, embassies and diplomatic corps:

1. Original application for connection (on standard Mobiuz letterhead, or on the organization’s letterhead) 2. Power of attorney for an authorized representative 3. Original passport of the authorized representative

For law enforcement agencies and security agencies:

1. Original application for connection (on standard Mobiuz letterhead, or on the organization’s letterhead).
